What we did
On August 23, 2026 we asked Google's AI Mode the same buyer question four different ways — "best," "top rated," "what is the best," and "most recommended" AI marketing agency for clinics and local service businesses — and recorded every business the answer cited with a link, in the order given.
Two things stood out immediately. First, the answer is never a single recommendation; every version of the question returned a categorised list. Second, the list is not stable. Only PatientGain appeared in all four answers. Cardinal Digital Marketing appeared in three, Thrive and Intrepy in two, and SmartSites, Inbound Medic, NoGood, Thryv and GoHighLevel appeared once each. If you are judging a vendor by whether an AI named them, run the question more than once — a single probe is close to a coin flip.
The two categories the AI describes
In its own framing, the market splits into vertical medical and local growth agencies that build compliance and patient-booking workflows, and AI-native platforms that give your team the tooling to do it. The answers also volunteered a third distinction we think matters more than either: whether a vendor is doing search visibility and AI visibility work at all, versus bolting AI writing onto a conventional Google and Facebook ads playbook.
That distinction is the useful one for a clinic owner, because the two failure modes look nothing alike. A platform fails when nobody on your team has the hours to operate it. An ads-first agency fails quietly, when your ad account performs and the assistant your patients now ask still has never heard of you.
How to choose between them
Ask any vendor on that list, or us, four questions. What exactly do you publish on my behalf, and where does it live — my domain or yours? How will you show me whether an AI assistant names my practice, in words I can read? Who does the work each week — your team or mine? And what happens in month four, when the easy wins are gone?
The answers separate vendors faster than a case study does. A platform will answer the third question with "your team." An ads-first agency will struggle with the second, because measuring an AI answer is a different job from reporting on a Google Ads dashboard.

That answer changes weekly, it varies by how the question is phrased, and it is now the first thing a patient reads. Measure it before you buy anything.

Is the answer the same every time you ask?
No. In our four probes only one firm appeared in all four answers, and the phrasing of the question changed which firms appeared. Anyone quoting a single AI answer as proof of standing is quoting a snapshot, not a ranking.
