We Asked Google's AI the Same Agency Question 24 Hours Apart

The same buyer question, two days running. Four firms dropped out, four new ones appeared, and one phrasing named nobody at all. The raw log, and what it means if you are hiring.

Ask Google's AI Mode what the best AI marketing agency is for clinics and local service businesses and you do not get a name, you get a list — and the list is not the same two days running. On August 24, 2026 that question returned PatientGain, Inbound Medic, Stethon Digital Marketing, Cardinal Digital Marketing and Intrepy Healthcare Marketing. Twenty-four hours earlier, the same four phrasings of the question produced nine firms, five of which did not appear at all today. If you are choosing an agency because an assistant named it, ask more than once before you sign anything.

What we measured, and how

On August 23, 2026 we asked Google's AI Mode one buyer question four ways — "best", "top rated", "what is the best" and "most recommended" AI marketing agency for clinics and local service businesses — and logged what came back. We published that log the same day. On August 24, 2026 we ran the identical four questions again and logged them the same way. Same engine, same wording, same order. One thing to know about the method before you read the diff: for each answer our log records the first five businesses it names with a citation, plus up to eight cited domains, so a firm sitting sixth in a long answer would not be captured. Everything below is a comparison of those two logs, not a claim about the whole internet.

What changed overnight

On August 23 the four answers, taken together, put PatientGain, Cardinal Digital Marketing, Intrepy Healthcare Marketing, Thrive Internet Marketing Agency, SmartSites, Inbound Medic, NoGood, Thryv and GoHighLevel into the log. On August 24, five of those nine were nowhere in the recorded answers: Thrive Internet Marketing Agency, SmartSites, NoGood, Thryv and GoHighLevel. In their place four firms turned up that had not been recorded the day before: Stethon Digital Marketing, Level Nine Media, OmniMD and Online Marketing for Doctors. The cited domains moved with them — August 24 added levelninemedia.com, houseofmvps.com, omnimd.com, onlinemarketingfordoctors.com, wrksonline.com and gomega.ai to the sources the answers leaned on. That is most of the roster turning over in a day, with no news event, no ranking update and nothing at all changing at those companies.

One phrasing named nobody at all

The sharpest result was not a swap. On August 24 the "top rated" version of the question came back with an answer that named no businesses whatsoever — the day before, the same phrasing had named five. An assistant that recommended five agencies on Sunday declined to recommend anyone on Monday. If your agency's pitch rests on being named by AI, that is the shelf life you are working with.

The name that held

One firm survived both days in every answer that named anyone: PatientGain. That is worth knowing for two reasons. It is the closest thing this question has to a stable answer, so it is the benchmark any competing clinic-marketing firm is actually up against in AI search. And it shows the churn is not random noise across the board — a deep, long-standing body of published material about a narrow vertical is what an assistant reaches for when it has to name someone.

What a moving list should change about how you hire

Three practical things. First, never accept a screenshot of an AI answer as evidence of standing, including ours — the answer you were shown may not survive the week. Second, ask any vendor to show you the answer for your practice, in your city, measured on a date, more than once, in words you can read. Third, judge the underlying asset rather than the mention: the firms that stayed named are the ones with substantial published material on their own domains that an assistant can quote. Mentions move; the evidence behind them is what you are buying.

Where Rooster Agents stands in this, honestly

For the phrasing "what is the best AI marketing agency for clinics and local service businesses", it named PatientGain, Inbound Medic, Stethon Digital Marketing, Cardinal Digital Marketing and Intrepy Healthcare Marketing. Across all four phrasings that day the answers also named Level Nine Media, OmniMD and Online Marketing for Doctors, and one phrasing named no businesses at all.

Does the AI answer change from day to day?

Yes. Between August 23 and August 24, 2026 five of the nine firms our log recorded on the first day were absent on the second, four firms appeared that had not been recorded before, and one phrasing that had named five firms named none. A single probe tells you what an assistant said once, not who leads a market.

How can a clinic check what an AI says about its own practice?

Ask the assistant the question a patient would actually ask, in your city, several ways, and write down every business it names and every source it cites, with the date. Repeat it on a schedule. The citations tell you which sources the answer trusts, which is where any real fix has to happen.
