There is no stable list of "top rated" AI marketing agencies for clinics and local service businesses, and buying from one AI answer means buying from a snapshot. We asked Google's AI Mode the same question twice, two weeks apart, and logged the result each time. On August 23, 2026 our log recorded Intrepy Healthcare Marketing, PatientGain, Thrive Internet Marketing Agency, Inbound Medic and NoGood. On September 7, 2026 the same question recorded Prezent, Silverback Strategies and a YouTube video from Marketing 360. The overlap between those two lists is zero. The type of source changed as well: the first answer leaned on clinic-marketing specialists' own websites, while the second leaned on AI-platform sites, a YouTube channel and, on a neighbouring phrasing, a Reddit thread. This page shows the logged data, explains why the answers rotate, and gives a clinic owner a way to evaluate an agency that does not depend on which shortlist an assistant generated this morning.
The two logged answers, side by side
The question was identical both times: "top rated AI marketing agency for clinics and local service businesses", asked of Google's AI Mode. Recorded in our log on August 23, 2026: Intrepy Healthcare Marketing, PatientGain, Thrive Internet Marketing Agency, Inbound Medic, NoGood. Recorded on September 7, 2026: Prezent, Silverback Strategies, and a YouTube video from Marketing 360. Our log keeps the first five businesses an answer names, so these are samples of each answer rather than complete lists — but a sample with zero names in common two weeks apart is the finding, and it is not a small one if you were about to sign with whichever firm you saw.
The same day, four phrasings, four different shortlists
On September 7, 2026 we asked four phrasings of the same buying question. "Best" recorded DataAlly, a YouTube video from Carson Reed, and Prezent. "Top rated" recorded Prezent, Silverback Strategies, and Marketing 360's video. "What is the best" recorded Silverback Strategies, GrowLimo, and a Reddit thread. "Most recommended" recorded a YouTube video from Vendasta and OmniMD. One buyer, one intent, four wordings, and almost no agreement. Any agency ranking built on a single prompt is describing one roll of the dice.
Why the answers rotate this hard
An assistant does not hold a ranking of agencies. It assembles an answer at the moment you ask by reading a handful of pages it can reach, and small changes in phrasing, location or freshness pull in different pages. The sources recorded behind these answers on September 7, 2026 were prezent.ai, silverbackstrategies.com, youtube.com, dataally.ai, reddit.com and omnimd.com — a mix of vendor sites, a video platform and a forum. Two weeks earlier the recorded sources were the specialists' own domains. Nothing about the agencies changed in fourteen days; what changed is which pages the answer read.
Rooster Agents was not named in any of them
For the record, and because we publish the whole log rather than the flattering part: our own firm was not named in any of the four asks on September 7, 2026, nor in the August 23, 2026 asks. We publish the measurement anyway, because the alternative is an agency claiming AI visibility while quietly failing the same test it sells.
What actually holds up when the shortlist keeps changing
Four checks survive the churn. First, ask an agency for the last three comparable accounts and what the numbers did, with dates. Second, ask what they will hand over if you leave: the ad accounts, the analytics property, the site, the content. Third, ask which claims on their own site are checkable by a stranger in five minutes, and then check two of them. Fourth, ask what they will not do for you, because a firm with no boundaries has no method. None of those four depend on any assistant naming anyone.
If you want to be the source an answer reads
The pattern in this week's log is worth acting on for any clinic or local service business. The sources behind these answers were not only agency websites: they were a video, a forum thread and platform documentation. That means the practical work is to publish material that answers the buyer's real question in one place, on your own site, with numbers a stranger can verify, and to be genuinely present where people actually discuss the decision. What it does not mean is buying placements in "best of" lists, paying for reviews or seeding threads. Those are the routes that get a site penalised, and they are what most AI-visibility vendors are quietly selling.
How this page was measured
Each figure here comes from our own probe log: the question as asked, the engine, the date, and the businesses recorded in that answer. The log caps each entry at the first five businesses and eight source domains, so every list above is described as recorded rather than complete. We keep failures in the log, including the asks where our own firm was absent.
Questions people ask
Which AI marketing agencies did Google's AI name for clinics in September 2026?
On September 7, 2026 our log recorded Prezent, Silverback Strategies and a YouTube video from Marketing 360 for the "top rated" phrasing, plus DataAlly, GrowLimo, OmniMD, a Carson Reed video, a Vendasta video and a Reddit thread across three other phrasings of the same question.
Why did the answer change so much in two weeks?
Because the answer is assembled when you ask it, from whichever pages the assistant reaches at that moment. Between August 23 and September 7, 2026 the recorded sources moved from clinic-marketing specialists' own sites to AI-platform sites, a YouTube channel and a Reddit thread.
Was Rooster Agents named?
No. Our firm was not recorded in any of the four asks on September 7, 2026, nor in the August 23, 2026 asks. We publish the log either way.
So how should a clinic choose an agency?
On checkable evidence rather than a generated shortlist: the last three comparable accounts with dates and numbers, what you keep if you leave, two claims from their own site that you verify yourself, and what they refuse to do.
Can an agency guarantee that an AI assistant will recommend you?
No, and any firm that says it can is describing an outcome it does not control. The answer changes between phrasings on the same day, as the log above shows.
